Advanced resorption can lead to the loss of the second molar \u2014 a functional and valuable tooth. When identified early, it warrants prompt extraction of the wisdom tooth. <\/p>\n\n

Extraction at the clinic \u2014 when Dr Zerguine refers<\/h3>\n\n

The vast majority of wisdom tooth extractions<\/a> are performed directly at the clinic under local anesthesia.<\/p>\n\n

The only situation in which Dr Zerguine refers you to an oral and maxillofacial surgeon \u2014 when the lower wisdom tooth is in close contact with the inferior alveolar nerve (formerly called the inferior dental nerve). This nerve runs through the lower jaw and supplies sensation to the lip and chin. Extracting under these conditions without specialized equipment carries a risk of nerve injury \u2014 paresthesia or permanent numbness of the lower lip. <\/p>\n\n

This risk is assessed on a panoramic X-ray or cone beam scan before any decision is made.<\/p>\n\n

What the assessment includes<\/h3>\n\n

Before any extraction decision \u2014 a complete clinical and radiographic assessment is essential.<\/p>\n\n

It determines the exact position of the tooth, its relationship to nearby anatomical structures, the risk of complications, and the optimal timing for intervention if extraction is indicated.<\/p>\n\n

Do all wisdom teeth need to be removed?<\/strong> No \u2014 a well-positioned, asymptomatic wisdom tooth that can be cleaned properly can be kept under regular radiographic monitoring.<\/p>\n\n
