A tooth breaking is always a worrying moment. The patient feels something snap, discovers a sharp edge with their tongue, or notices a missing piece when looking in the mirror. The first question that comes to mind is: Is it serious? Do I need to consult immediately? Is the tooth lost? <\/p>\n\n

The answer depends on several clinical criteria that I will explain here, based on my daily practice at Dental Swiss Clinics<\/a> in Montreux.<\/p>\n\n

Always provide same-day care<\/h3>\n\n

The principle guiding my practice is simple: a broken tooth is seen at the clinic the same day. Always. <\/p>\n\n

This does not mean that all treatment must be definitive on the same day. It means that a clinical examination must be done quickly to assess the severity, provide relief if necessary, and decide on the course of action. <\/p>\n\n

The patient’s apprehension about a broken tooth is legitimate. They cannot know on their own if the situation requires urgent intervention or can wait a few days. It is up to the practitioner to make this assessment, and to do it quickly. <\/p>\n\n

What I assess during the first examination<\/h3>\n\n

Before proposing treatment, several elements must be checked. They determine the nature of the care and its urgency. <\/p>\n\n

The location of the fracture<\/strong>Does the fracture only concern the visible crown, or does it affect the pulp (the nerve)? A fracture that exposes the pulp requires a different response than a superficial fracture. <\/p>\n\n

The type of tooth<\/strong>An incisor, a premolar, a molar\u2014each has its specificities. Incisors often pose an immediate aesthetic problem. Molars pose a functional and occlusal problem. Premolars are in between. <\/p>\n\n

The occlusal context<\/strong>Does the patient clench or grind their teeth<\/a>? Teeth grinding during sleep generates forces that may have contributed to the fracture, and which can generate others. This information changes the reconstruction strategy. <\/p>\n\n

The tooth’s previous condition<\/strong>Has this tooth already undergone root canal treatment? Has this tooth already been restored<\/a> in the past? Is this the first time it has broken, or is it a repeated episode? A tooth that breaks for the second time raises a fundamental question about its ability to be preserved long-term. <\/p>\n\n

The periodontal and bone condition<\/strong>A complete clinical evaluation, supplemented if necessary by an X-ray, allows us to check if an infection is present at the root or bone level. A deep bone infection can completely redefine the clinical priority: in this case, the visible fracture is no longer the central issue; it is the infectious focus that must be treated. <\/p>\n\n

This assessment takes a few minutes during the consultation. It is what allows us to propose a clear course of action, without haste or underestimation. <\/p>\n\n

Clinical approaches based on severity<\/h3>\n\n

Crown fracture without pulp involvement<\/strong>This is the most common situation. The tooth is broken, but the pulp (nerve) is not exposed. Treatment can be a direct composite restoration, or an indirect restoration such as a ceramic crown made in-office in one visit<\/a>. The choice depends on the extent of the fracture. <\/p>\n\n

Fracture with pulp involvement<\/strong>The pulp is exposed. The response depends on how long the exposure has been present and the pulpal status. Prompt management can sometimes preserve the tooth’s vitality. In other cases, root canal treatment becomes necessary, followed by an appropriate restoration. <\/p>\n\n

Fracture with root involvement<\/strong>This is the most complex situation. Vertical root fractures are notoriously difficult to treat and often compromise tooth preservation. A precise radiographic diagnosis is essential for making a decision. <\/p>\n\n

Fracture on a root-canal-treated tooth<\/strong>A tooth that has already undergone root canal treatment and then fractures is more fragile. Its preservation depends on the exact location of the fracture and the remaining strength of the tooth structure. <\/p>\n\n

When the tooth cannot be saved<\/h3>\n\n

In my practice, approximately 1 in 10 fractures results in extraction. This is not rare, nor is it frequent. Some fractures make preservation impossible or impractical. <\/p>\n\n

The criteria that guide this decision are clear: a deep vertical root fracture, a fracture that extends below the bone level, an extensive infection around the root, or tooth destruction too significant to allow for reliable reconstruction.<\/p>\n\n

This conversation with the patient is always delicate. No one likes to hear that a tooth needs to be extracted. My role is to clearly explain why preservation is not possible or not reasonable, what replacement solutions are available (implant, bridge, or sometimes abstention), and to let the patient make an informed decision. <\/p>\n\n

An extraction is rarely an absolute emergency. It can be planned over a few days, allowing the patient time to process the situation and choose a replacement solution. <\/p>\n\n

What matters for preserving the tooth<\/h3>\n\n

A few simple actions help preserve the tooth while awaiting consultation.<\/p>\n\n

Do not chew on the side of the broken tooth. Avoid very hot or very cold foods. If a piece of tooth has broken off, keep it in a little milk or a damp compress, and bring it to the consultation: in some cases, it can be reattached. <\/p>\n\n

Avoid aggressive actions on the area (vigorous brushing, toothpicks, insistent exploration with the tongue). A weakened tooth can break further. <\/p>\n\n

Frequently Asked Questions<\/h3>\n\n

Does a broken tooth always hurt?<\/strong>No. A superficial fracture that does not reach the pulp can be completely painless. This is even common. The absence of pain should not lead to delaying consultation, as the fracture can evolve and secondarily expose the pulp. <\/p>\n\n

Can a piece of tooth be reattached?<\/strong>In some cases, yes. If the fragment is clean, preserved in good condition, and brought in quickly, it can be durably reattached. This is more common for incisor fractures than for others. <\/p>\n\n

Should I consult urgently even without pain?<\/strong>Yes, the same day. A clinical evaluation is necessary to determine the severity and prevent the situation from worsening without the patient realizing it. <\/p>\n\n

How many broken teeth end up being extracted?<\/strong>About 1 in 10 in my practice. The majority of broken teeth can be saved, provided they are managed quickly and rigorously evaluated. <\/p>\n\n

What to do if the tooth breaks in the evening or on the weekend?<\/strong>Contact the clinic by phone<\/a> or WhatsApp<\/a>. A dental emergency during opening hours can be handled the same day.
