It is always a sign of inflammation, and this inflammation deserves to be understood and managed. <\/p>\n\n<p class=\"wp-block-paragraph\">Here is how I approach this subject at <a href=\"https:\/\/dentalswiss-clinics.ch\/en\/\">Dental Swiss Clinics<\/a> in Montreux.<\/p>\n\n<h3 id=\"h-le-saignement-est-un-signe-d-inflammation\" class=\"wp-block-heading\">Bleeding is a sign of inflammation<\/h3>\n\n<p class=\"wp-block-paragraph\">Gingival bleeding is one of the classic signs of inflammation. This inflammation can be mild and reversible (early-stage gingivitis), or more advanced and require more in-depth care. <\/p>\n\n<h3 id=\"h-la-cause-la-plus-frequente-l-insuffisance-de-nettoyage-interdentaire\" class=\"wp-block-heading\">The most frequent cause: insufficient interdental cleaning<\/h3>\n\n<p class=\"wp-block-paragraph\">In my practice, the most frequent cause of gingival bleeding is not a lack of brushing, but a lack or absence of cleaning between the teeth.<\/p>\n\n<p class=\"wp-block-paragraph\">Classic brushing, no matter how rigorous, only cleans a portion of the tooth surfaces. Scientific literature documents that it removes about 42% of dental biofilm. The rest, particularly in interdental spaces, requires specific tools.  <\/p>\n\n<p class=\"wp-block-paragraph\">Three options are available, to be adapted to each situation:<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Dental floss<\/strong><br\/>Suitable for narrow interdental spaces in patients with healthy gums or early-stage inflammation. Requires technical learning to be effective. <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Interdental brushes<\/strong><br\/>Recent literature documents them as more effective than floss for reducing plaque and gingival inflammation. Suitable for wider spaces, particularly in patients with bridges, crowns, or spaces created by dental maturation. <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Water flossers<\/strong><br\/>Useful as a supplement, especially for patients with manual dexterity difficulties or those wearing orthodontic appliances. Evidence of effectiveness is more limited than for brushes or floss, but it remains a good complementary tool. <\/p>\n\n<p class=\"wp-block-paragraph\">The choice of tool depends on each patient&#8217;s clinical situation. This choice is discussed during a consultation, based on the morphology of the spaces, dexterity, and personal preferences. <\/p>\n\n<h3 id=\"h-l-evolution-avec-l-age\" class=\"wp-block-heading\">Evolution with age<\/h3>\n\n<p class=\"wp-block-paragraph\">A point rarely explained to patients, but important to understand.<\/p>\n\n<p class=\"wp-block-paragraph\">In the general population, we observe an evolution of dental problems with age. From childhood until about age 30, cavities dominate. After 30, gum problems progressively become the primary concern.  <\/p>\n\n<p class=\"wp-block-paragraph\">This evolution is explained by several factors. Gingival tissues change with age. Dental maturation sometimes creates interdental spaces that did not exist in youth. The accumulation of tartar over the years requires regular maintenance.   <\/p>\n\n<p class=\"wp-block-paragraph\">About 30% of the adult population over 50 is affected by periodontal disease. This is not inevitable; it is an epidemiological reality that justifies special attention beyond the age of thirty. <\/p>\n\n<h3 id=\"h-ma-demarche-face-a-un-saignement\" class=\"wp-block-heading\">My approach to bleeding<\/h3>\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/dentalswiss-clinics.ch\/en\/care-treatments\/\">My clinical protocol<\/a> when facing a patient with gingival bleeding always follows the same logic.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>One, professional intervention<\/strong><br\/>A complete clinical examination is performed, with an evaluation of the gingival status, periodontal probing if necessary, and X-rays to check the underlying bone condition. This check-up ensures that the inflammation is well under control, that hygiene habits are well integrated, and allows for maintenance scaling if necessary. <\/p>\n\n<p class=\"wp-block-paragraph\">This three-step approach yields reliable results in the vast majority of early-stage gingivitis situations.<\/p>\n\n<h3 id=\"h-quand-une-prise-en-charge-plus-approfondie-est-necessaire\" class=\"wp-block-heading\">When more in-depth care is necessary<\/h3>\n\n<p class=\"wp-block-paragraph\">Certain situations require going beyond this standard care.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Deep periodontal pockets<\/strong><br\/>When probing reveals pockets deeper than 4 mm, it is no longer simple gingivitis but early or established periodontitis. More in-depth care is necessary, sometimes with the intervention of a periodontist. <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Bone loss visible on X-ray<\/strong><br\/>Damage to the alveolar bone signals a progression that goes beyond simple gingival inflammation. Management becomes more complex. <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Rapid recurrence despite improved habits<\/strong><br\/>If bleeding persists or recurs quickly despite improved hygiene and scaling, a more thorough evaluation is required.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Specific contexts<\/strong><br\/>Certain general situations (pregnancy, diabetes, medical treatments) modify the gingival response and require adapted attention.<\/p>\n\n<p class=\"wp-block-paragraph\">In these situations, collaboration with a periodontist can be organized, particularly for cases requiring periodontal surgery or prolonged specialized follow-up.<\/p>\n\n<h3 id=\"h-ce-qu-il-ne-faut-pas-faire\" class=\"wp-block-heading\">What not to do<\/h3>\n\n<p class=\"wp-block-paragraph\">Three frequent errors I encounter during consultations.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>One, trivializing the bleeding<\/strong><br\/>&#8220;It&#8217;s normal when I brush hard&#8221; is a phrase to forget. Bleeding always deserves attention\u2014not necessarily major worry, but at least an understanding and adapted actions. <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Two, stopping brushing in the bleeding area<\/strong><br\/>The first reflex for some patients is to avoid the bleeding area to prevent further irritation. It is the opposite that must be done: the bleeding area is the one that needs cleaning the most, using the correct technique. Bleeding generally decreases within days of implementing proper hygiene.  <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Three, using antiseptic mouthwashes as a long-term solution<\/strong><br\/>Antiseptic mouthwashes can help occasionally, but they do not replace mechanical cleaning. Their prolonged use can even disrupt the oral bacterial balance. <\/p>\n\n<h3 id=\"h-questions-frequentes\" class=\"wp-block-heading\">Frequently Asked Questions<\/h3>\n\n<p class=\"wp-block-paragraph\"><strong>Is occasional bleeding serious?<\/strong><br\/>It is not necessarily serious, but it is never normal. It signals inflammation that deserves to be understood. A consultation allows the cause to be determined and appropriate steps to be taken.  <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>How long does it take for bleeding to disappear after improving hygiene?<\/strong><br\/>Generally a few days to two weeks.
