The “every six months” rule is repeated like a dogma, even though it doesn’t match every patient’s clinical reality.

My position at Dental Swiss Clinics in Montreux is clear: scaling frequency must be tailored to each situation. A patient with excellent hygiene and a healthy periodontium does not require the same schedule as a patient with an inflammatory tendency or periodontal involvement.

Understanding what tartar is

Before talking about frequency, you need to understand what scaling treats.

Daily brushing removes dental plaque, a soft bacterial biofilm that constantly deposits on teeth. As long as plaque remains soft, it can be removed by brushing and interdental cleaning.

The problem starts when plaque isn’t removed. After 48 hours, it begins to mineralize. The scientific literature confirms that half of mineralization occurs in the first two days, and that the full process can take up to 12 days.

Once mineralized, plaque becomes tartar. Tartar is a hard, adherent deposit that cannot be removed by brushing. Only professional scaling can eliminate it.

The speed of tartar formation varies from one patient to another. Saliva quality plays a major role: salivary calcium and phosphate concentrations, and the presence of natural mineralization inhibitors, are individual. Some patients form very little tartar; others accumulate it quickly despite good hygiene.

My frequency framework in practice

My decision on frequency is based on each patient’s clinical assessment, not a general rule.

Once a year when the situation is stable
A patient with healthy gums, no inflammation, and appropriate hygiene with regular, effective interdental cleaning (floss or interdental brushes depending on the case) can stick to yearly scaling. This is the ideal situation, and it applies to about one third of my patients.

Every six months in cases of gingivitis or insufficient interdental cleaning
When I observe gingival inflammation, or when interdental cleaning isn’t yet effective, I schedule the next check-up at six months. This interval allows us to confirm that hygiene measures have been properly adopted and that inflammation is under control. This frequency is reassessed at the next visit: if everything is stable, we can move to once a year.

Every four months in cases of periodontitis
When the periodontal situation is compromised, with deep periodontal pockets or documented bone loss, the interval becomes shorter. A four-month interval helps maintain control of inflammation and prevent disease progression. This frequency is also reassessed regularly based on how things evolve.

Why the general “every six months” rule isn’t enough

This rule has the advantage of simplicity, but it treats all patients the same even though their needs are very different.

A low periodontal-risk patient with rigorous hygiene does not need to be seen every six months. Overtreating minimal tartar has little clinical value.

A high-risk patient with established periodontitis needs to be seen more often than every six months. Too long an interval allows the disease to progress silently.

Individual tailoring is what truly protects the patient’s health.

The vicious circle: plaque, tartar, inflammation

One point patients often misunderstand concerns the perceived aggressiveness of scaling. Some patients feel sensitivity during or after the procedure and conclude that scaling has “damaged” their teeth. The reality is different.

When a periodontal situation sets in, a vicious circle develops. Tartar accumulates. It sustains inflammation. Inflammation worsens periodontitis. Periodontitis creates more space for tartar. Brushing becomes painful, which further reduces hygiene effectiveness. The cycle becomes self-perpetuating.

When we intervene in this situation with scaling, we remove deposits that were shielding certain tooth surfaces that have become sensitive. The sensitivity that appears isn’t caused by scaling—it is revealed by it.

That is why early scaling, before periodontitis becomes established, is more comfortable than late scaling. Preventing the vicious circle is easier than breaking it once it is in place.

What scaling does not do

Three common misconceptions need to be corrected.

Scaling is not whitening
Scaling removes tartar deposits and some superficial stains linked to diet, tobacco, or tea. It makes teeth cleaner, but it does not change their intrinsic shade. A patient who wants true lightening should opt for a specific teeth whitening treatment.

Scaling does not damage enamel
When performed properly, with appropriate instruments (ultrasonic scaling and fine hand instrumentation), scaling does not damage tooth structure. The “aggressive” sensation some patients perceive comes from exposing surfaces already sensitized by underlying periodontal disease—not from the procedure itself.

Scaling does not replace brushing
Professional scaling once or twice a year never compensates for inadequate daily brushing. It’s the at-home hygiene routine that protects teeth and gums between visits. Scaling treats what daily hygiene could not prevent.

How the practice is organized

A useful clarification about how roles are divided in the practice.

Scaling itself is performed by the dental hygienist or the prophylaxis assistant. They are professionals specifically trained for this procedure, both technical and educational. They carry out the cleaning, provide tailored hygiene advice, and assess each patient’s progress over successive visits.

The dental treatment plan, however, is the dentist’s responsibility. Scaling and the treatment plan are two independent procedures. A patient can come in for scaling without committing to a treatment plan, or vice versa.

This professional division is clear and respects each person’s scope of practice. It allows the patient to benefit from coordinated care without role confusion.

Frequently Asked Questions

How often should you have scaling?
It depends on your clinical situation. Once a year if hygiene is excellent and the periodontium is healthy; every six months in cases of an inflammatory tendency or insufficient interdental cleaning; every four months in cases of periodontitis. An in-consultation assessment determines the appropriate interval.

Is scaling painful?
Usually not. Some patients feel sensitivity during the procedure, particularly when periodontal inflammation is present. This sensitivity decreases once the situation is stabilized with regular scaling and appropriate hygiene.

How long does a scaling appointment take?
Generally between 30 and 60 minutes, depending on the clinical situation and the amount of tartar to remove.

Does scaling whiten teeth?
No. It removes tartar deposits and some superficial stains, but it does not change the intrinsic shade of the teeth. Whitening is a separate treatment.

Does scaling damage enamel?
No, when it is performed properly. The “aggressive” sensation some patients perceive comes from exposing surfaces already sensitized by underlying inflammation—not from the procedure itself.

For an assessment of your situation and tailored scheduling, contact Dental Swiss Clinics in Montreux: Monday 8 AM–8 PM, Tuesday to Friday 8 AM–6 PM.

Scaling frequency depends on each patient’s clinical situation.
